Care Coordination services are particularly impactful for clients with complex mental, chemical, and physical health care needs.

Alluma’s Care Coordinators:

• Work with a multidisciplinary team of providers within Alluma and throughout the community
• Review provider recommendations with the client and answer questions about Alluma services
• Identify and address barriers to care
• Help clients navigate healthcare pathways, connecting to resources within Alluma and in the community

In addition, our Corrections Care Coordinator works within Tri-County Community Corrections (TCCC) to coordinate care and provide crisis services inside the jail.
